prabhura eka bhakta — ‘dvija kamalākānta’ nāma
tāṅre lañā nīlācale karilā prayāṇa
Synonyms: 
prabhura — of Śrī Caitanya Mahāprabhu; eka bhakta — one devotee; dvija kamalākānta — Dvija Kamalākānta; nāma — named; tāṅre — him; lañā — accepting as his companion; nīlācale — to Jagannātha Purī; karilā — did; prayāṇa — departure.
Translation: 
There was a devotee of Śrī Caitanya Mahāprabhu’s named Dvija Kamalākānta, whom Paramānanda Purī took with him to Jagannātha Purī.
